Anxiety and depression in IBD: what the research shows
Inflammatory bowel disease is measured in the gut: calprotectin, endoscopy, biopsies. The way it is lived includes a great deal that those measurements miss, and anxiety and depression are among the best-studied parts of that gap.
How common it is
A systematic review and meta-analysis in The Lancet Gastroenterology & Hepatology set out to estimate the prevalence of anxiety and depression symptoms in people with inflammatory bowel disease [6]. A pooled estimate like this describes a population, not an individual, but the fact that it needed a meta-analysis of its own shows how established the question has become.
Quality of life moves with the disease
A systematic review and meta-analyses of quality of life in IBD examined how the condition affects day-to-day wellbeing across studies [12]. A separate systematic review asked how people with IBD cope with the disease [14], an important complement because coping is something a person can work on, unlike the course of the disease itself.
One stressor comes up repeatedly in patients' own accounts: unpredictability. Not knowing when the next flare will come, or where the nearest toilet is, can weigh on someone even when their symptoms are currently controlled.
Treatment goals now include how you feel
STRIDE-II, the international consensus on treatment targets for IBD from the International Organization for the Study of IBD, sets the goals for "treat-to-target" care [7]. Treat-to-target means setting measurable goals and adjusting treatment until they are met, rather than accepting partial control.
Psychological care has its own guidance too. The Spanish Working Group on Crohn's Disease and Ulcerative Colitis (GETECCU), together with the patients' association ACCU, published recommendations on managing psychological problems in people with IBD [11]. A joint clinician and patient recommendation is a clear signal that this is part of care, not an optional extra.
What to bring up, and with whom
| If you notice… | It is worth saying to… |
|---|---|
| Persistent low mood or loss of interest | Your GP or IBD team; ask whether psychological support is part of the service |
| Anxiety centred on flares, toilets or travel | Your IBD nurse; this is a commonly reported stressor |
| Fatigue that makes everything harder | Your IBD team; see this site's living with IBD page |
| Symptoms that feel out of step with your test results | Your gastroenterologist; both sides of the picture count |
A worked example
Someone newly diagnosed with Crohn's disease has well-controlled symptoms but spends a lot of energy planning around the possibility of a flare. Because unpredictability is a recognised stressor in coping research, and psychological care has dedicated guidance in IBD, it is reasonable to raise this with the care team directly rather than wait for it to become severe.
Common questions
Is depression in IBD caused by the disease or by living with a chronic illness?
Will controlling the inflammation improve my mood?
Should I be screened?
Where can I get help urgently?
