Çfarë shkakton IBD? Gjenetika, mjedisi, incidenca në rritje
As gjenetika as mjedisi nuk e shpjegojnë vetë sëmundjen inflamatore të zorrëve. Gjenetika të tregon kush është i predispozuar; epidemiologjia të tregon se predispozita vetëm nuk mund të jetë e gjithë historia, sepse harta globale e sëmundjes ka lëvizur tepër shpejt për t'u shpjeguar nga gjenet.
Çfarë tregon vërtet gjenetika
Studimet e asociimit në të gjithë genomin kanë identifikuar një numër të konsiderueshëm lokusesh të lidhura me predispozitën për IBD. Një meta-analizë e të dhënave të asociimit identifikoi 38 lokuse predispozicioni dhe theksoi rrezik gjenetik të përbashkët mes popullatave dhe, në mënyrë domethënëse, pjesërisht të përbashkët me sëmundje të tjera me ndërmjetësim imunitar [25]. Një studim tjetër, i veçantë, asociimi në të gjithë genomin implikoi aktivizimin imunitar të disa geneve integrina në veçanti [24] — një gjetje interesante nga pikëpamja mekanike, pasi barnat bllokuese të integrinave (klasa biologjike anti-integrinë e trajtuar në faqen tonë të trajtimit) veprojnë duke ndërprerë pikërisht shtegun e trafikimit të leukociteve ku ndodhen këto gene.
Përfundimi praktik nga gjenetika është më i ngushtë nga sa presin njerëzit. IBD nuk është sëmundje me një gen të vetëm, e trashëguar sipas Mendelit — është poligjenike, me shumë lokuse që secili kontribuon një sasi të vogël rreziku. Të kesh një të afërm të shkallës së parë me IBD e rrit rrezikun mbi popullatën e përgjithshme, por shumica dërrmuese e njerëzve që bartin variante gjenetike të lidhura me IBD nuk e zhvillojnë kurrë sëmundjen. Gjenetika vendos një prag predispozicioni; diçka tjetër vendos nëse ai prag kapërcehet.
Pse gjenetika nuk mund të jetë e gjithë historia
Arkitektura gjenetike e një popullate nuk ndryshon brenda pak dekadash. Incidenca globale e IBD-së ka ndryshuar. Një analizë e vitit 2025 në Nature gjurmon incidencën e IBD-së nëpër ato që i quan faza epidemiologjike — incidencë e ulët, një fazë përshpejtimi kur incidenca rritet ndjeshëm, më pas një fazë ekuilibri kompozicional prevalence ku incidenca stabilizohet por numri i njerëzve që jetojnë me sëmundjen vazhdon të rritet sepse është kronike [35]. Rajonet që po industrializohen sot po kalojnë nëpër fazën e përshpejtimit që Evropa Perëndimore dhe Amerika e Veriut e kaluan gjatë shekullit të njëzetë.
Ky model u dokumentua më herët në një shqyrtim sistematik të studimeve të bazuara në popullatë mbi incidencën dhe prevalencën në mbarë botën [23], dhe një shqyrtim tjetër që trajton veçanërisht epidemiologjinë përgjatë moshave në epokën e terapive të avancuara e vendos pamjen aktuale globale [33]. Një shqyrtim sistematik i fokusuar te IBD me fillim pediatrik gjeti të njëjtin trend përshpejtues veçanërisht te fëmijët dhe adoleshentët [15], dhe trendet e shtrimit në spital gjatë së njëjtës periudhë u karakterizuan në një shqyrtim tjetër sistematik me analiza kohore [7].
Një model gjeografik dhe kohor që ndjek industrializimin dhe jo shmangien gjenetike (genetic drift) është argumenti më i fortë i disponueshëm se diçka lidhur me mënyrën se si jeton një popullatë — jo thjesht si lind — po bën një pjesë të konsiderueshme të punës shkakësore.
Kandidatët mjedisorë
Një shqyrtim ombrellë i meta-analizave ekzaminoi drejtpërdrejt faktorët e rrezikut mjedisor për IBD, duke bashkuar evidencën nga shumë studime individuale ekspozimi në një sintezë të vetme [20]. Përmbledhja e ndershme e kësaj literature është se disa kandidatë kanë evidencë mbështetëse me forcë të ndryshme, dhe asnjëri nuk është treguar i mjaftueshëm më vete — në përputhje me një sëmundje që me sa duket kërkon që një individ i predispozuar gjenetikisht të hasë një kombinim të veçantë ekspozimesh.

Pse ka rëndësi praktike kjo
Asgjë nga kjo nuk ndryshon menaxhimin. Nuk identifikon një ushqim që duhet shmangur, një ndryshim stili jetese që garanton parandalimin e shpërthimeve, apo një test që parashikon rrezikun individual me saktësi të dobishme. Çfarë shpjegon në të vërtetë janë dy gjëra që pacientëve u thuhen shpesh në mënyrë jo të qëndrueshme: se IBD nuk "shkaktohet" nga dieta apo stresi në ndonjë kuptim të thjeshtë (gjenetika dhe ndryshimet mjedisore në nivel popullate po bëjnë shumë më tepër punë shkakësore sesa zgjedhjet e përditshme), dhe se nuk është as fat i keq thjesht gjenetik (incidenca në rritje në rajonet e industrializuara rishtazi e përjashton këtë vetvetiu). Të dyja këto rikonceptime priren të ulin fajin që pacientët raportojnë rreth pyetjes "çfarë bëra unë që e shkaktova këtë" — një pyetje që baza e evidencës në fakt nuk mbështet t'i bëhet asnjë sjelljeje të vetme.
