Сравнение на биологични и напреднали терапии
За пръв път сравнителният въпрос — кое лекарство, а не дали лекарство — има мета-аналитични отговори.
Какво се промени
В продължение на две десетилетия базата от доказателства се основаваше на сравнения лекарство срещу плацебо, които показваха на клиницистите, че биологичните лекарства действат, но не давали информация кое от тях да се избере. Мрежовият мета-анализ промени това, използвайки общи компаратори за индиректно класиране на терапиите [1][4].
Практическото следствие е, че изборът на терапия от първа линия вече може да се обосновава, а не да се прави по подразбиране, като разликите между отделните агенти по отношение на индукция спрямо поддържане, и на ефикасност спрямо безопасност, стават видими.

Лечение по цел, не по симптоми
Най-важната промяна в лечението на ВЗЧ е разбирането, че симптомите са слаб показател за възпалението. Хората могат да се чувстват добре при активно възпаление и да се чувстват зле в ремисия. Затова настоящите европейски препоръки се фокусират върху обективни маркери — фекален калпротектин, ендоскопско зарастване — а не върху това как пациентът описва усещанията си.
Това е и отговорът на въпроса за храненето, поставен на страницата за диетата: интервенция, която подобрява симптомите, без да подобрява обективните маркери, не е лекувала болестта.
Когато едно лекарство не е достатъчно
Двойната терапия с биологични лекарства или комбинираната нискомолекулна терапия е оценена при рефрактерно заболяване [3]. Тя остава решение на специалист, основано на ограничена база от доказателства, и е областта, в която изследванията на едноклетъчно ниво, характеризиращи хетерогенността на отговора [2], най-вероятно ще променят практиката — чрез предвиждане на кой механизъм трябва да бъде прицелен при конкретния пациент, вместо да се открива чрез последователни неуспехи на лечението.
