Biologics en geavanceerde therapieën vergeleken
Voor het eerst heeft de vergelijkende vraag — welke medicatie, niet of een medicatie — meta-analytische antwoorden.
Wat is veranderd
Twee decennia lang was de bewijsbasis medicijn versus placebo, wat clinici vertelde dat biologics werken en niets over welke te kiezen. Netwerkmeta-analyse heeft dat veranderd door gedeelde vergelijkers te gebruiken om therapieën indirect te rangschikken [1][4].
De praktische consequentie is dat de keuze voor de eerste lijn nu beredeneerd kan worden in plaats van standaard, en verschillen tussen middelen op inductie versus onderhoud, en op effectiviteit versus veiligheid, zichtbaar zijn.

Behandelen op doel, niet op symptomen
De meest ingrijpende verschuiving in IBD-beheer is dat symptomen een slechte proxy zijn voor ontsteking. Mensen voelen zich goed met actieve ontsteking en voelen zich onwel in remissie. Huidige Europese richtlijnen richten zich daarom op objectieve markers — fecale calprotectine, endoscopische genezing — in plaats van hoe de patiënt zich voelt.
Dit is ook het antwoord op de dieetvraag die op de dieetpagina is gesteld: een interventie die symptomen verbetert zonder objectieve markers te verbeteren, heeft de ziekte niet behandeld.
Wanneer één medicijn niet genoeg is
Dual biologic of gecombineerde kleine-molecuultherapie is beoordeeld voor refractaire ziekte [3]. Het blijft een specialistische beslissing met een beperkte bewijsbasis, en het is waar single-cell werk dat responsheterogeniteit karakteriseert [2] het meest waarschijnlijk de praktijk zal veranderen — door te voorspellen welk mechanisme te richten bij een bepaalde patiënt in plaats van het te ontdekken door sequentieel falen.
