Vermoeidheid

Vermoeidheid is een van de meest gemelde symptomen bij IBD en een van de minst behandelde. Ze blijft aanwezig bij een groot deel van de mensen wier ontsteking onder controle is, wat betekent dat het niet simpelweg een marker van ziekteactiviteit is.

Bijdragende factoren zijn onder meer anemie (ijzertekort komt heel vaak voor en is te corrigeren), verstoorde slaap, medicatie-effecten, chronische ontsteking en de psychologische last van een onvoorspelbare ziekte. Het is de moeite waard om de corrigeerbare factoren actief aan te pakken, want niemand zal ze voor u aankaarten tijdens een afspraak van vijftien minuten die draait om calprotectine.

Geestelijke gezondheid

De percentages angst en depressie bij IBD liggen aanzienlijk hoger dan de achtergrondpercentages. De voor de hand liggende verklaring — leven met een onvoorspelbare, gestigmatiseerde ziekte die soms incontinentie veroorzaakt, is belastend — klopt, maar is waarschijnlijk niet het hele verhaal, gezien de inflammatoire en gut-brain-routes die worden beschreven op onze bewijssite over de hersen-darmas.

Wat in de praktijk telt, is dat dit behandelbaar is en routinematig wordt gemist. Psychologische ondersteuning maakt deel uit van de IBD-zorg volgens Europese richtlijnen, maar wordt ongelijk aangeboden.

De praktische last

  • Angst om geen toilet te vinden, die reizen, werk en sociaal leven meer beperkt dan artsen doorgaans beseffen.
  • Onvoorspelbaarheid. Plannen is moeilijk wanneer een opflakkering zonder waarschuwing kan optreden.
  • Voedingsbeheer. Individuele triggers, geen gevalideerde lijst en veel tegenstrijdig advies — zie voeding.
  • Werk. Vermoeidheid en aandrang gaan slecht samen met starre werkpatronen.
  • Reizen. Opslag van medicatie, voedselveiligheid en toegang — beschreven op onze reisgezondheidssite.

Epidemiologie

De incidentie van IBD is hoog en stabiel in Europa en Noord-Amerika, en stijgt sterk in recent geïndustrialiseerde landen. Dat geografische patroon is een van de sterkste argumenten voor een omgevingscomponent — voeding, hygiëne, antibiotica en het microbioom zijn allemaal kandidaten, en geen daarvan is bewezen.

Waarom ben ik uitgeput als ik in remissie ben?
Vermoeidheid blijft vaak bestaan nadat de ontsteking onder controle is. Controleer eerst ferritine en B12; tekorten hierin komen vaak voor, zijn te corrigeren en worden vaak over het hoofd gezien.
Veroorzaakt stress mijn opflakkeringen?
Stress heeft mogelijk invloed op de symptomen, en het bewijs dat het ontsteking veroorzaakt is zwakker dan het volksmodel suggereert. Het is niet uw schuld.
Heb ik een operatie nodig?
Een aanzienlijke minderheid heeft dat wel nodig, en dat aandeel is gedaald doordat effectieve therapie eerder wordt ingezet — zie behandeling.
Kan ik reizen?
