Únava

Únava patrí medzi najčastejšie hlásené symptómy pri IBD a medzi najmenej často liečené. Pretrváva u veľkej časti ľudí, ktorých zápal je kontrolovaný, čo znamená, že to nie je len marker aktivity ochorenia.

Prispievatelia zahŕňajú anémiu (nedostatok železa je veľmi bežný a opraviteľný), narušenie spánku, účinky liekov, chronický zápal a psychologickú záťaž z nepredvídateľnej choroby. Opravy hodné sú explicitne sledované, pretože nikto ich za vás nevyzdvihne na pätnásťminútovom stretnutí zameranom na kalprotektín.

Duševné zdravie

Úrovne úzkosti a depresie pri IBD podstatne prevyšujú základné úrovne. Zjavné vysvetlenie — žiť s nepredvídateľnou, stigmatizovanou, niekedy inkontinenciu spôsobujúcou chorobou je stresujúce — je reálne a pravdepodobne nie je celým príbehom, vzhľadom na zápalové a črevno-mozgové dráhy pokryté na našej gut-brain evidence site.

Prakticky dôležité je, že toto je liečiteľné a bežne prehliadané. Psychologická podpora je súčasťou starostlivosti o IBD v európskych usmerneniach a je nerovnomerne poskytovaná.

Praktická záťaž

  • Úzkosť z prístupu na toaletu, ktorá obmedzuje cestovanie, prácu a spoločenský život viac, než si klinici zvyčajne uvedomujú.
  • Nepredvídateľnosť. Plánovanie je ťažké, keď môže prísť zhoršenie bez varovania.
  • Správa diéty. Individuálne spúšťače, žiadny overený zoznam a množstvo protichodných rád — pozri diet.
  • Zamestnanie. Únava a naliehavosť zle interagujú s rigidnými pracovnými vzorcami.
  • Cestovanie. Uloženie liekov, bezpečnosť potravín a prístup — pokryté na našej travel health site.

Epidemiológia

Incidencia IBD je vysoká a stabilná v Európe a Severnej Amerike a prudko narastá v novo industrializovaných krajinách. Tento geografický vzor je jedným z najsilnejších argumentov pre environmentálnu zložku — diéta, hygiena, antibiotiká a mikrobiom sú všetci kandidáti, a žiadny nie je etablovaný.

Prečo som vyčerpaný/á, keď som v remisii?
Únava často pretrváva aj po zvládnutí zápalu. Najprv skontrolujte feritín a vitamín B12; ich nedostatok je bežný, dá sa napraviť a často sa prehliada.
Spôsobuje moje vzplanutia stres?
Stres pravdepodobne ovplyvňuje príznaky, no dôkazy o tom, že spôsobuje zápal, sú slabšie, než hovorí ľudová predstava. Nie je to vaša vina.
Budem potrebovať operáciu?
Značná menšina áno, pričom tento podiel klesol s tým, ako sa účinná liečba nasadzuje skôr — pozrite si liečbu.
Môžem cestovať?
Áno, s plánovaním okolo liekov, poistenia a bezpečnosti potravín.
