Nuovargis

Nuovargis yra vienas iš dažniausiai pranešamų simptomų sergant IBD ir vienas iš mažiausiai gydomų. Jis išlieka didelėje dalyje žmonių, kurių uždegimas yra kontroliuojamas, tai reiškia, kad tai nėra tiesiog ligos aktyvumo rodiklis.

Priežastys apima anemiją (geležies trūkumas yra labai dažnas ir taisomas), miego sutrikimus, vaistų poveikį, lėtinį uždegimą ir psichologinę naštą dėl nenuspėjamos ligos. Taisytini dalykai yra verti aiškaus siekimo, nes niekas jų nepakels per penkiolikos minučių konsultaciją, orientuotą į kalprotektiną.

Psichinė sveikata

Nerimo ir depresijos rodikliai sergant IBD žymiai viršija fono rodiklius. Akivaizdi priežastis — gyventi su nenuspėjama, stigmatizuota, kartais šlapimo nelaikymą sukeliančia liga yra varginanti — yra tikra ir greičiausiai ne visa istorija, atsižvelgiant į uždegiminius ir žarnyno-smegenų kelius, aprašytus mūsų žarnyno-smegenų įrodymų svetainėje.

Praktiškai svarbu, kad tai yra gydoma ir dažnai praleidžiama. Psichologinė parama yra IBD priežiūros dalis Europos gairėse ir teikiama nevienodai.

Praktinė našta

  • Tualeto prieigos nerimas, kuris apriboja keliones, darbą ir socialinį gyvenimą labiau, nei paprastai vertina klinikai.
  • Nenuspėjamumas. Planavimas yra sudėtingas, kai paūmėjimas gali atsirasti be įspėjimo.
  • Dietos valdymas. Individualūs sukėlėjai, nėra patvirtinto sąrašo ir daug prieštaringų patarimų — žr. dietą.
  • Darbas. Nuovargis ir skubėjimas blogai sąveikauja su griežtomis darbo schemomis.

Epidemiologija

IBD incidencija Europoje ir Šiaurės Amerikoje yra didelė ir stabili, o naujai industrializuotose šalyse sparčiai auga. Šis geografinis modelis yra vienas iš stipriausių argumentų už aplinkos komponentą — dieta, higiena, antibiotikai ir mikrobiomas yra visi kandidatai, ir nė vienas nėra patvirtintas.

Dažnai užduodami klausimai

Kodėl aš esu išsekęs, jei esu remisijoje?
Nuovargis dažnai išlieka po uždegimo kontrolės. Pirmiausia patikrinkite ferritiną ir B12; jie yra dažni, taisomi ir dažnai praleidžiami.
Ar stresas sukelia mano paūmėjimus?
Stresas tikėtina, kad veikia simptomus, o įrodymai, kad jis sukelia uždegimą, yra silpnesni nei liaudies modelis. Tai ne jūsų kaltė.
Ar man reikės operacijos?
Reikšminga mažuma to reikia, o proporcija sumažėjo, kai veiksminga terapija buvo taikoma anksčiau — žr. gydymą.
Ar galiu keliauti?
Taip, su planavimu aplink vaistus, draudimą ir maisto saugumą.
