Þreyta

Þreyta er meðal þeirra einkenna sem oftast eru skráð í IBD og meðal þeirra sem sjaldan eru meðhöndluð. Hún viðvarandi í stórum hluta fólks þar sem bólgan er stjórnað, sem þýðir að hún er ekki einfaldlega merki um sjúkdómsvirkni.

Þættir sem stuðla að þessu eru blóðleysi (járnskortur er mjög algengur og hægt að leiðrétta), truflun á svefni, áhrif lyfja, langvarandi bólga, og andlegt álag vegna óútreiknanlegs sjúkdóms. Þeir sem hægt er að leiðrétta eru þess virði að skoða sérstaklega, því enginn mun taka þá upp fyrir þig í fimmtán mínútna viðtali sem einblínir á calprotectin.

Andleg heilsa

Kvíði og þunglyndi í IBD eru verulega hærri en bakgrunnstíðni. Augljós skýringin — að lifa með óútreiknanlegum, stimpluðum, stundum þvagfalla-sjúkdómi er sársaukafull — er raunveruleg og líklega ekki öll sagan, miðað við bólgu- og þarma-heila leiðirnar sem fjallað er um á okkar þarma-heila sönnunarsíðu.

Það sem skiptir máli í reynd er að þetta er meðhöndlanlegt og oft misskilinn. Andleg stuðningur er hluti af IBD umönnun samkvæmt evrópskum leiðbeiningum og er ójafnt veittur.

Praktískt álag

  • Kvíði vegna aðgangs að salernum, sem takmarkar ferðalög, vinnu og félagslíf meira en læknar venjulega átta sig á.
  • Óútreiknanleiki. Að skipuleggja er erfitt þegar skyndileg uppsveifla getur komið án fyrirvara.
  • Fæðustjórnun. Einstaklinga-örvandi efni, engin staðfest listi, og mikið af mótsagnakenndum ráðleggingum — sjá fæði.
  • Atvinna. Þreyta og bráð þörf hafa slæm áhrif á stífa vinnuskipulag.
  • Ferðalög. Lyfjageymsla, matvælaöryggi og aðgangur — fjallað um á okkar ferðaheilsusíðu.

Epidemiology

Tíðni IBD er há og stöðug í Evrópu og Norður-Ameríku og hækkar skarpt í ný iðnvæddum löndum. Þessi landfræðilegi mynstur er eitt af sterkustu rökum fyrir umhverfisþætti — fæði, hreinlæti, sýklalyf og örveruflóra eru allt frambjóðendur, og enginn er staðfestur.

Af hverju er ég þreyttur ef ég er í remissíu?
Þreyta viðvarandi oft lengra en bólgu stjórn. Athugaðu ferritín og B12 fyrst; þau eru algeng, leiðréttanleg og oft misskilin.
Er streita að valda uppsveiflum mínum?
Streita hefur líklega áhrif á einkenni og sönnunin um að hún valdi bólgu er veikari en þjóðsagan. Það er ekki þín sök.
Þarf ég að fara í skurðaðgerð?
