Træthed

Træthed er blandt de mest hyppigt rapporterede symptomer ved IBD og blandt de mindst hyppigt behandlede. Den vedvarer hos en stor del af de personer, hvis inflammation er kontrolleret, hvilket betyder, at det ikke blot er en markør for sygdomsaktivitet.

Bidragende faktorer inkluderer anæmi (jernmangel er meget almindelig og korrigerbar), søvnforstyrrelser, medicinens virkninger, kronisk inflammation og den psykologiske belastning ved en uforudsigelig sygdom. De korrigerbare faktorer er værd at forfølge eksplicit, fordi ingen vil tage dem op for dig i en femten-minutters aftale fokuseret på calprotectin.

Trætheden ved IBD

Mental sundhed

Angst- og depressionsraterne ved IBD overstiger betydeligt baggrundsraterne. Den åbenlyse forklaring — at leve med en uforudsigelig, stigmatiseret, nogle gange inkontinensfremkaldende sygdom er stressende — er reel og sandsynligvis ikke hele historien, givet de inflammatoriske og tarm-hjerne-veje, der er dækket på vores gut-brain evidence site.

Det, der praktisk set betyder noget, er, at dette er behandlingsbart og rutinemæssigt overset. Psykologisk støtte er en del af IBD-plejen i europæiske retningslinjer og leveres ujævnt.

Den praktiske byrde

  • Toiletadgangsangst, som begrænser rejser, arbejde og socialt liv mere end klinikere typisk værdsætter.
  • Uforudsigelighed. Planlægning er svær, når et skub kan komme uden varsel.
  • Koststyring. Individuelle triggere, ingen valideret liste, og en masse modstridende råd — se diet.
  • Ansættelse. Træthed og hastværk interagerer dårligt med stive arbejdsformer.

Epidemiologi

IBD-forekomsten er høj og stabil i Europa og Nordamerika og stiger kraftigt i nyindustrielle lande. Det geografiske mønster er et af de stærkeste argumenter for en miljømæssig komponent — kost, hygiejne, antibiotika og mikrobiomet er alle kandidater, og ingen er etableret.

Almindelige spørgsmål

Hvorfor er jeg udmattet, hvis jeg er i remission?
Træthed vedvarer ofte efter inflammatorisk kontrol. Tjek ferritin og B12 først; de er almindelige, korrigerbare og ofte overset.
Forårsager stress mine skub?
Stress påvirker plausibelt symptomer, og beviset for, at det forårsager inflammation, er svagere end folkemodellen. Det er ikke din skyld.
Vil jeg få brug for kirurgi?
En betydelig minoritet gør, og andelen er faldet, da effektiv terapi kom tidligere — se treatment.
Kan jeg rejse?
Ja, med planlægning omkring medicin, forsikring og fødevaresikkerhed.
