Viver com IBD: fadiga, dor e partes não medidas

A remissão no papel e a remissão vivida são coisas diferentes, e a lacuna é onde a maior parte do fardo reside.

Fadiga

A fadiga está entre os sintomas mais frequentemente relatados na DII e entre os menos frequentemente tratados. Ela persiste em uma grande fração de pessoas cuja inflamação está controlada, o que significa que não é simplesmente um marcador da atividade da doença.

Os contribuintes incluem anemia (a deficiência de ferro é muito comum e corrigível), distúrbios do sono, efeitos de medicamentos, inflamação crônica e a carga psicológica de uma doença imprevisível. Os fatores corrigíveis valem a pena serem perseguidos explicitamente, porque ninguém os levantará para você em uma consulta de quinze minutos focada na calprotectina.

Saúde mental

As taxas de ansiedade e depressão na DII superam substancialmente as taxas de fundo. A explicação óbvia — viver com uma doença imprevisível, estigmatizada e, às vezes, causadora de incontinência é angustiante — é real e provavelmente não é toda a história, dado os caminhos inflamatórios e intestino-cérebro abordados em nosso site de evidências intestino-cérebro.

O que importa na prática é que isso é tratável e rotineiramente negligenciado. O suporte psicológico é parte do cuidado da DII nas orientações europeias e é entregue de forma desigual.

O fardo prático

  • Ansiedade de acesso ao banheiro, que restringe viagens, trabalho e vida social mais do que os clínicos normalmente apreciam.
  • Imprevisibilidade. Planejar é difícil quando uma crise pode chegar sem aviso.
  • Gestão da dieta. Gatilhos individuais, nenhuma lista validada e uma grande quantidade de conselhos contraditórios — veja dieta.
  • Emprego. A fadiga e a urgência interagem mal com padrões de trabalho rígidos.
  • Viagem. Armazenamento de medicamentos, segurança alimentar e acesso — abordados em nosso site de saúde de viagem.

Epidemiologia

A incidência de DII é alta e estável na Europa e na América do Norte e está a aumentar acentuadamente em países recém-industrializados. Esse padrão geográfico é um dos argumentos mais fortes para um componente ambiental — dieta, higiene, antibióticos e o microbioma são todos candidatos, e nenhum está estabelecido.

A fadiga persiste frequentemente para além do controlo da inflamação. Verifique primeiro a ferritina e a vitamina B12; são causas comuns, corrigíveis e muitas vezes ignoradas.

É plausível que o stress influencie os sintomas, e a evidência de que causa inflamação é mais fraca do que o modelo popular sugere. Não é culpa sua.
