Vivir con EII: fatiga, dolor y lo no medido

La remisión en papel y la remisión como se experimenta son cosas diferentes, y la brecha es donde vive la mayor parte de la carga.

Fatiga

La fatiga está entre los síntomas más frecuentemente reportados en la EII y entre los menos frecuentemente tratados. Persiste en una gran fracción de personas cuya inflamación está controlada, lo que significa que no es simplemente un marcador de actividad de la enfermedad.

Los contribuyentes incluyen anemia (la deficiencia de hierro es muy común y corregible), interrupción del sueño, efectos de medicamentos, inflamación crónica y la carga psicológica de una enfermedad impredecible. Los que son corregibles valen la pena perseguir explícitamente, porque nadie los mencionará por ti en una cita de quince minutos centrada en la calprotectina.

Salud mental

Las tasas de ansiedad y depresión en la EII superan sustancialmente las tasas de fondo. La explicación obvia — vivir con una enfermedad impredecible, estigmatizada y a veces causante de incontinencia es angustiante — es real y probablemente no es toda la historia, dado los caminos inflamatorios y del eje intestino-cerebro cubiertos en nuestro sitio de evidencia del eje intestino-cerebro.

Lo que importa prácticamente es que esto es tratable y se pasa por alto rutinariamente. El apoyo psicológico es parte de la atención de la EII en la guía europea y se entrega de manera desigual.

La carga práctica

  • Ansiedad por el acceso al baño, que restringe los viajes, el trabajo y la vida social más de lo que los clínicos suelen apreciar.
  • Imprevisibilidad. Planificar es difícil cuando un brote puede llegar sin previo aviso.
  • Manejo de la dieta. Disparadores individuales, sin lista validada, y una gran cantidad de consejos contradictorios — ver dieta.
  • Empleo. La fatiga y la urgencia interactúan mal con patrones de trabajo rígidos.
  • Viajes. Almacenamiento de medicamentos, seguridad alimentaria y acceso — cubierto en nuestro sitio de salud en viajes.

Epidemiología

La incidencia de la EII es alta y estable en Europa y América del Norte y está aumentando drásticamente en países recién industrializados. Ese patrón geográfico es uno de los argumentos más fuertes para un componente ambiental — la dieta, la higiene, los antibióticos y el microbioma son todos candidatos, y ninguno está establecido.

Preguntas frecuentes

¿Por qué estoy agotado si estoy en remisión?
La fatiga suele persistir más allá del control de la inflamación. Compruebe primero la ferritina y la B12; son causas frecuentes, corregibles y a menudo pasadas por alto.
¿El estrés causa mis brotes?
Es plausible que el estrés influya en los síntomas, pero la evidencia de que cause inflamación es más débil de lo que sugiere la creencia popular. No es culpa suya.
¿Necesitaré cirugía?
Una minoría considerable sí la necesita, y esa proporción ha disminuido a medida que los tratamientos eficaces llegan antes — véase tratamiento.
