Fatiga

La fatiga és un dels símptomes més freqüentment informats en IBD i dels menys tractats. Persisteix en una gran fracció de persones la inflamació de les quals està controlada, cosa que significa que no és simplement un marcador de l'activitat de la malaltia.

Els factors contribuidors inclouen l'anèmia (la deficiència de ferro és molt comú i corregible), la interrupció del son, els efectes dels medicaments, la inflamació crònica i la càrrega psicològica d'una malaltia imprevisible. Els que són corregibles valen la pena perseguir-los explícitament, perquè ningú els plantejarà per tu en una cita de quinze minuts centrada en la calprotectina.

Les taxes d'ansietat i depressió en IBD superen substancialment les taxes de fons. L'explicació òbvia — viure amb una malaltia imprevisible, estigmatitzada i que a vegades causa incontinència és estressant — és real i probablement no és tota la història, donada les vies inflamatòries i intestinals cobertes al nostre lloc d'evidència intestí-cervell.

El que importa pràcticament és que això és tractable i habitualment es passa per alt. El suport psicològic és part de l'atenció a IBD en les guies europees i es proporciona de manera desigual.

La càrrega pràctica

  • Ansietat per l'accés al lavabo, que restringeix viatges, treball i vida social més del que els clínics solen apreciar.
  • Imprevisibilitat. Planificar és difícil quan una crisi pot arribar sense avís.
  • Gestió de la dieta. Triggers individuals, sense llista validada, i una gran quantitat de consells contradictoris — vegeu dieta.
  • Empleament. La fatiga i l'urgència interactuen malament amb patrons de treball rígids.
  • Viatge. Emmagatzematge de medicaments, seguretat alimentària i accés — cobert al nostre lloc de salut viatgera.

Epidemiologia

La incidència d'IBD és alta i estable a Europa i Amèrica del Nord i està augmentant dràsticament en països recentment industrialitzats. Aquest patró geogràfic és un dels arguments més forts per a un component ambiental — la dieta, la higiene, els antibiòtics i el microbioma són tots candidats, i cap està establert.

Per què estic esgotat si estic en remissió?
La fatiga sovint persisteix més enllà del control inflamatori. Comprova ferritina i B12 primer; són comuns, corregibles i sovint es passen per alt.
L'estrès està causant les meves crisis?
L'estrès plausiblement influeix en els símptomes i l'evidència que causa inflamació és més feble que el model popular. No és culpa teva.
Necessitaré cirurgia?
Una minoria substancial ho fa, i la proporció ha disminuït a mesura que la teràpia efectiva ha arribat més aviat — vegeu tractament.
