Vivre avec la MII : fatigue, douleur et les aspects que personne ne mesure
La rémission sur papier et la rémission vécue sont des choses différentes, et l'écart est là où se trouve la majeure partie du fardeau.
Fatigue
La fatigue est l'un des symptômes les plus fréquemment rapportés dans la MII et l'un des moins fréquemment traités. Elle persiste chez une grande fraction de personnes dont l'inflammation est contrôlée, ce qui signifie qu'elle n'est pas simplement un marqueur de l'activité de la maladie.
Les contributeurs incluent l'anémie (la carence en fer est très courante et corrigible), les perturbations du sommeil, les effets des médicaments, l'inflammation chronique et la charge psychologique d'une maladie imprévisible. Les facteurs corrigibles valent la peine d'être poursuivis explicitement, car personne ne les soulèvera pour vous lors d'un rendez-vous de quinze minutes axé sur la calprotectine.

Santé mentale
Les taux d'anxiété et de dépression dans la MII dépassent considérablement les taux de fond. L'explication évidente — vivre avec une maladie imprévisible, stigmatisée, parfois causant de l'incontinence est éprouvant — est réelle et probablement pas toute l'histoire, compte tenu des voies inflammatoires et intestino-cérébrales couvertes sur notre site de preuves intestino-cérébrales.
Ce qui compte pratiquement, c'est que cela est traitable et souvent négligé. Le soutien psychologique fait partie des soins de la MII dans les recommandations européennes et est délivré de manière inégale.
Le fardeau pratique
- Anxiété d'accès aux toilettes, qui contraint les voyages, le travail et la vie sociale plus que ce que les cliniciens apprécient généralement.
- Imprévisibilité. La planification est difficile lorsqu'une poussée peut survenir sans avertissement.
- Gestion du régime alimentaire. Déclencheurs individuels, pas de liste validée, et beaucoup de conseils contradictoires — voir régime.
- Emploi. La fatigue et l'urgence interagissent mal avec des modèles de travail rigides.
- Voyage. Stockage des médicaments, sécurité alimentaire et accès — couvert sur notre site de santé des voyageurs.
Épidémiologie
L'incidence de la MII est élevée et stable en Europe et en Amérique du Nord et augmente rapidement dans les pays nouvellement industrialisés. Ce schéma géographique est l'un des arguments les plus solides en faveur d'un composant environnemental — le régime alimentaire, l'hygiène, les antibiotiques et le microbiome sont tous des candidats, et aucun n'est établi.
