Umor

Umor je jedan od najčešće prijavljivanih simptoma u IBD-u i među najmanje često tretiranim. Održava se u velikom dijelu ljudi čija je upala kontrolisana, što znači da nije samo marker aktivnosti bolesti.

Doprinosi uključuju anemiju (nedostatak željeza je vrlo čest i ispravljiv), poremećaj sna, efekte lijekova, hroničnu upalu i psihološki teret nepredvidive bolesti. Ispravni uzroci su vrijedni eksplicitnog rješavanja, jer ih niko neće spomenuti na petnaestominutnom sastanku fokusiranom na kalprotectin.

Mentalno zdravlje

Stope anksioznosti i depresije u IBD-u znatno premašuju osnovne stope. Očigledno objašnjenje — život s nepredvidivom, stigmatiziranom, ponekad inkontinencijom uzrokovanom bolešću je stresan — je stvarno i vjerovatno nije cijela priča, s obzirom na upalne i crijevno-mozgane puteve pokrivene na našoj stranici o dokazima crijevno-mozgane osi.

Ono što je praktično važno je da je ovo tretabilno i rutinski propušteno. Psihološka podrška je dio njege IBD-a u evropskim smjernicama i nerazmjerno se pruža.

Praktični teret

  • Anksioznost zbog pristupa toaletu, koja ograničava putovanja, rad i društveni život više nego što kliničari obično cijene.
  • Nepredvidivost. Planiranje je teško kada iznenada može doći do pogoršanja.
  • Upravljanje dijetom. Individualni okidači, bez validirane liste, i mnogo kontradiktornih savjeta — vidi dijetu.
  • Zapošljavanje. Umor i hitnost loše se uklapaju u krute radne obrasce.
  • Putovanje. Skladištenje lijekova, sigurnost hrane i pristup — pokriveno na našoj stranici o zdravstvenom putovanju.

Epidemiologija

Incidencija IBD-a je visoka i stabilna u Evropi i Sjevernoj Americi i naglo raste u novim industrijaliziranim zemljama. Taj geografski obrazac je jedan od najjačih argumenata za ekološku komponentu — dijeta, higijena, antibiotici i mikrobiom su svi kandidati, a nijedan nije utvrđen.

Zašto sam iscrpljen ako sam u remisiji?
Umor često potraje i nakon što se upala stavi pod kontrolu. Prvo provjerite feritin i B12; njihov nedostatak je čest, popravljiv i često se previdi.
Da li stres izaziva moje pogoršanje bolesti?
Stres vjerovatno utiče na simptome, a dokazi da izaziva upalu slabiji su od uvriježenog narodnog shvatanja. To nije vaša krivica.
Da li će mi biti potrebna operacija?
Znatnoj manjini jeste, a taj udio se smanjio otkako se efikasna terapija primjenjuje ranije — pogledajte liječenje.
