Umor

Umor je jedan od najčešće prijavljenih simptoma kod IBD-a i među najmanje često tretiranim. Održava se kod velikog dela ljudi čija je upala kontrolisana, što znači da nije samo marker aktivnosti bolesti.

Faktori uključuju anemiju (nedostatak gvožđa je veoma čest i može se ispraviti), poremećaj sna, efekte lekova, hroničnu upalu i psihološki teret nepredvidive bolesti. Oni koji se mogu ispraviti su vredni eksplicitnog istraživanja, jer ih niko neće pomenuti na petnaestominutnom sastanku fokusiranom na kalprotektin.

Mentalno zdravlje

Stope anksioznosti i depresije kod IBD-a znatno premašuju osnovne stope. Očigledno objašnjenje — život sa nepredvidivom, stigmatizovanom, ponekad inkontinencijom uzrokovanom bolešću je stresan — je stvarno i verovatno nije cela priča, s obzirom na upalne i crevno-mozgane puteve pokrivene na našem sajtu o dokazima crevno-mozgane osi.

Ono što je praktično važno je da je ovo tretabilno i rutinski propušteno. Psihološka podrška je deo brige o IBD-u u evropskim smernicama i nerazmerno se pruža.

Praktični teret

  • Anksioznost zbog pristupa toaletu, koja ograničava putovanja, rad i društveni život više nego što to kliničari obično shvataju.
  • Nepredvidivost. Planiranje je teško kada iznenada može doći do pogoršanja.
  • Upravljanje dijetom. Individualni okidači, bez validirane liste, i mnogo kontradiktornih saveta — vidi dijetu.
  • Zapošljavanje. Umor i hitnost loše deluju sa strogim radnim obrascima.
  • Putovanje. Čuvanje lekova, bezbednost hrane i pristup — pokriveno na našem sajtu o zdravstvenom putovanju.

Epidemiologija

Incidencija IBD-a je visoka i stabilna u Evropi i Severnoj Americi i naglo raste u novim industrijalizovanim zemljama. Taj geografski obrazac je jedan od najsnažnijih argumenata za postojanje ekološke komponente — dijeta, higijena, antibiotici i mikrobiom su svi kandidati, a nijedan nije utvrđen.

Zašto sam iscrpljen ako sam u remisiji?
Umor često opstaje nakon kontrole upale. Proverite feritin i B12 prvo; oni su česti, ispravni i često propušteni.
Da li stres uzrokuje moja pogoršanja?
Stres verovatno utiče na simptome, a dokazi da uzrokuje upalu su slabiji od narodnog modela. Nije vaša krivica.
Da li će mi biti potrebna operacija?
