Vivere con IBD: affaticamento, dolore e le parti che nessuno misura

Affaticamento

L'affaticamento è uno dei sintomi più frequentemente segnalati nell'IBD e tra i meno frequentemente trattati. Persiste in una grande frazione di persone la cui infiammazione è controllata, il che significa che non è semplicemente un indicatore dell'attività della malattia.

I fattori contributivi includono l'anemia (la carenza di ferro è molto comune e correggibile), la distorsione del sonno, gli effetti dei farmaci, l'infiammazione cronica e il carico psicologico di una malattia imprevedibile. Quelli correggibili meritano di essere perseguiti esplicitamente, perché nessuno li solleverà per te in un appuntamento di quindici minuti focalizzato sulla calprotectina.

Salute mentale

I tassi di ansia e depressione nell'IBD superano sostanzialmente i tassi di base. La spiegazione ovvia — vivere con una malattia imprevedibile, stigmatizzata e talvolta causa di incontinenza è angosciante — è reale e probabilmente non è tutta la storia, date le vie infiammatorie e intestino-cervello trattate nel nostro sito sulle evidenze intestino-cervello.

Ciò che conta praticamente è che questo è trattabile e frequentemente trascurato. Il supporto psicologico è parte della cura dell'IBD nelle linee guida europee ed è fornito in modo disomogeneo.

Il carico pratico

  • Ansia per l'accesso al bagno, che limita i viaggi, il lavoro e la vita sociale più di quanto i clinici apprezzino normalmente.
  • Imprevedibilità. Pianificare è difficile quando una riacutizzazione può arrivare senza preavviso.
  • Gestione della dieta. Trigger individuali, nessun elenco validato e una grande quantità di consigli contraddittori — vedi dieta.
  • Occupazione. L'affaticamento e l'urgenza interagiscono male con schemi di lavoro rigidi.
  • Viaggio. Conservazione dei farmaci, sicurezza alimentare e accesso — trattati nel nostro sito sulla salute in viaggio.

Epidemiologia

L'incidenza dell'IBD è alta e stabile in Europa e Nord America e in forte aumento nei paesi recentemente industrializzati. Quel modello geografico è uno dei più forti argomenti per un componente ambientale — dieta, igiene, antibiotici e microbioma sono tutti candidati, e nessuno è stabilito.

L'affaticamento persiste frequentemente oltre il controllo infiammatorio. Controlla prima ferritina e B12; sono comuni, correggibili e spesso trascurati.

Lo stress plausibilmente influenza i sintomi e le evidenze che causa infiammazione sono più deboli del modello popolare. Non è colpa tua.
Avrò bisogno di un intervento chirurgico?
