Únava

Únava patří mezi nejčastěji hlášené symptomy u IBD a mezi nejméně často léčené. Přetrvává u velké části lidí, jejichž zánět je kontrolován, což znamená, že to není jen marker aktivity onemocnění.

Příčinami jsou anémie (nedostatek železa je velmi běžný a opravitelné), narušení spánku, účinky léků, chronický zánět a psychologická zátěž nepředvídatelného onemocnění. Duševní zdraví

Míra úzkosti a deprese u IBD výrazně překračuje základní míry. Očividné vysvětlení — žít s nepředvídatelným, stigmatizovaným, někdy inkontinenci způsobujícím onemocněním je stresující — je reálné a pravděpodobně ne celým příběhem, vzhledem k zánětlivým a střevně-mozkovým cestám pokrytým na našem gut-brain evidence site.

Prakticky důležité je, že to je léčitelné a rutinně přehlížené. Psychologická podpora je součástí péče o IBD v evropských pokynech a je poskytována nerovnoměrně.

Praktická zátěž

  • Úzkost z přístupu na toaletu, která omezuje cestování, práci a společenský život více, než si klinici obvykle uvědomují.
  • Nepředvídatelnost. Plánování je obtížné, když může flare přijít bez varování.
  • Správa diety. Individuální spouštěče, žádný ověřený seznam a spousta protichůdných rad — viz dieta.
  • Zaměstnání. Únava a naléhavost špatně interagují s rigidními pracovními vzorci.

Epidemiologie

Incidence IBD je vysoká a stabilní v Evropě a Severní Americe a prudce roste v nově industrializovaných zemích. Tento geografický vzor je jedním z nejsilnějších argumentů pro environmentální komponentu — dieta, hygiena, antibiotika a mikrobiom jsou všichni kandidáti, a žádný není potvrzen.

Proč jsem unavený, když jsem v remisi?
Únava často přetrvává i po kontrole zánětu. Nejprve zkontrolujte ferritin a B12; jsou běžné, opravitelné a často přehlížené.
Způsobuje stres mé flary?
Stres pravděpodobně ovlivňuje symptomy a důkazy, že způsobuje zánět, jsou slabší než lidový model. Není to vaše chyba.
Budu potřebovat operaci?
Podstatná menšina ano, a podíl klesl, jak efektivní terapie přišla dříve — viz léčba.
Mohu cestovat?
