Umor

Umor je jedan od najčešće prijavljenih simptoma u IBD-u i među najmanje često tretiranim. Perzistira u velikom dijelu ljudi čija je upala kontrolirana, što znači da nije samo marker aktivnosti bolesti.

Doprinosi uključuju anemiju (nedostatak željeza je vrlo čest i ispravljen), poremećaj spavanja, učinke lijekova, kroničnu upalu i psihološki teret nepredvidljive bolesti. Ispravni uzroci vrijedi izričito istražiti, jer ih nitko neće spomenuti tijekom petnaestominutnog pregleda usredotočenog na kalprotektin.

Mentalno zdravlje

Stope anksioznosti i depresije u IBD-u znatno premašuju pozadinske stope. Očito objašnjenje — život s nepredvidljivom, stigmatiziranom, ponekad inkontinencijom uzrokovanom bolešću je stresan — je stvarno i vjerojatno nije cijela priča, s obzirom na upalne i crijevno-mozgane putove pokrivene na našoj web stranici o crijevno-mozganoj povezanosti.

Ono što je praktično važno je da je ovo liječivo i rutinski propušteno. Psihološka podrška je dio skrbi za IBD prema europskim smjernicama i nejednako se pruža.

Praktični teret

  • Anksioznost zbog pristupa WC-u, koja ograničava putovanja, rad i društveni život više nego što to kliničari obično cijene.
  • Nepredvidljivost. Planiranje je teško kada iznenada može doći do pogoršanja.
  • Upravljanje prehranom. Individualni okidači, bez potvrđene liste, i mnogo kontradiktornih savjeta — pogledajte prehranu.
  • Zapošljavanje. Umor i hitnost loše se uklapaju u krute radne obrasce.

Epidemiologija

Incidencija IBD-a je visoka i stabilna u Europi i Sjevernoj Americi te naglo raste u novim industrijaliziranim zemljama. Taj geografski obrazac jedan je od najjačih argumenata za okolišni čimbenik — prehrana, higijena, antibiotici i mikrobiom su svi kandidati, a nijedan nije utvrđen.

Zašto sam iscrpljen ako sam u remisiji?
Umor često perzistira nakon upalne kontrole. Prvo provjerite feritin i B12; oni su česti, ispravni i često propušteni.
Uzrokuje li stres moja pogoršanja?
Stres vjerojatno utječe na simptome, a dokazi da uzrokuje upalu su slabiji od narodnog modela. Nije vaša krivnja.
Hoću li trebati operaciju?
