Pela primeira vez, a questão comparativa — qual medicamento, não se um medicamento — tem respostas meta-analíticas.
O que mudou
Durante duas décadas, a base de evidências foi medicamento versus placebo, que disse aos clínicos que os biológicos funcionam e nada sobre qual escolher. A meta-análise em rede mudou isso ao usar comparadores compartilhados para classificar terapias indiretamente [1][4].
A consequência prática é que a escolha de primeira linha agora pode ser raciocinada em vez de ser uma decisão padrão, e as diferenças entre agentes na indução versus manutenção, e na eficácia versus segurança, são visíveis.

Tratar para alvo, não para sintomas
A mudança mais consequente na gestão da DII é que os sintomas são um pobre proxy para a inflamação. As pessoas se sentem bem com inflamação ativa e se sentem mal em remissão. A orientação atual da Europa, portanto, visa marcadores objetivos — calprotectina fecal, cicatrização endoscópica — em vez de como o paciente relata se sentir.
Esta também é a resposta à questão dietética levantada na página da dieta: uma intervenção que melhora os sintomas sem melhorar marcadores objetivos não tratou a doença.
Quando um medicamento não é suficiente
A terapia biológica dupla ou combinada de pequenas moléculas foi avaliada para doença refratária [3]. Permanece uma decisão especializada com uma base de evidências limitada, e é onde o trabalho de célula única caracterizando a heterogeneidade da resposta [2] é mais provável de mudar a prática — prevendo qual mecanismo direcionar em um determinado paciente em vez de descobri-lo por falha sequencial.
