Cosa supporta effettivamente l'evidenza
Una sostanziale minoranza di pazienti con IBD riporta un miglioramento dei sintomi, ma mancano evidenze controllate per un effetto sull'infiammazione. [1]
La celiachia è più comune nell'IBD rispetto alla popolazione generale e dovrebbe essere esclusa prima di rimuovere il glutine, perché i test diventano inaffidabili una volta che si smette di mangiarlo.
Dove si colloca
L'evidenza è meccanicistica o osservazionale. Le persone riportano benefici, e questo vale la pena prenderlo sul serio, ma il beneficio riportato in una malattia a decorso recidivante-remittente è esattamente ciò che un'osservazione non controllata non può distinguere dalla fluttuazione naturale.
Come classifichiamo questi
Quattro classificazioni grossolane piuttosto che un punteggio. "Stabilito" significa trial randomizzati o approvazione delle linee guida. "Promettente" significa un segnale coerente da trial che sono piccoli o brevi. "Limitato" significa solo evidenza meccanicistica o osservazionale. "Popolare ma debole" significa ampiamente utilizzato e poco supportato — una categoria che esiste perché fingere che quegli approcci non siano presenti non aiuta nessuno.
