Does diet treat inflammatory bowel disease?

One dietary intervention induces remission about as well as steroids. Most of the others are far less supported than the internet suggests.

Exclusive enteral nutrition

A liquid formula diet, taken exclusively for six to eight weeks, induces remission in paediatric Crohn's disease at rates comparable to corticosteroids — without the growth suppression that makes steroids problematic in children. It is first-line in much of European paediatric practice.

Why it works is genuinely unclear. Candidate explanations include removing dietary antigens, changing the microbiome, removing food additives, and simply providing complete nutrition. That uncertainty matters, because it is why nobody can tell you which parts of a normal diet to remove to get the same effect.

Enteral nutrition for remission induction

Fibre: contested, and not the way people assume

A systematic review and meta-analysis examined high-fibre diets in Crohn's disease [2]. The picture is more favourable than the traditional low-residue advice implied — but with an important exception that gets lost in summaries.

In active disease with stricturing, bulky insoluble fibre can precipitate obstruction. That is a mechanical problem, not an inflammatory one, and it is why blanket advice fails. In quiescent disease without strictures, the general fibre evidence applies — see our gut-brain evidence site.

The FODMAP row deserves emphasis because it is the most misunderstood. Low-FODMAP diets can meaningfully reduce bloating and pain in IBD — and they do not reduce inflammation. Feeling better while inflammation continues is a genuinely dangerous outcome, which is why symptom-guided dietary management must sit alongside objective monitoring rather than replace it.

Trigger foods

Systematic review of food intolerance and exclusion diets in IBD finds what patients already know: triggers are real, individual, and inconsistent between people. There is no validated list. The only reliable method is systematic personal tracking — which is a data problem rather than a knowledge problem, and the reason a scanning app that learns your own pattern is more useful than a printed leaflet.

Common questions

Can I treat my IBD with diet alone?
Exclusive enteral nutrition can induce remission, mainly in paediatric Crohn's. No ordinary diet has been shown to replace maintenance therapy.
Should I avoid fibre?
Not routinely. Meta-analysis is more favourable than old advice [2] — with a real exception for stricturing disease.
Do food intolerance blood tests help?
IgG food testing has no validity for identifying triggers and reliably produces unnecessary restriction.
Is a low-FODMAP diet worth trying?
For symptoms, possibly, with dietitian support and reintroduction. It does not treat inflammation.
