Deiet
A yw diet yn trin clefyd llid y coluddyn?
Mae un ymyrraeth ddeietegol yn achosi maddeuant bron cystal â steroidau. Mae'r rhan fwyaf o'r lleill yn llawer llai wedi'u cefnogi na'r hyn y mae'r rhyngrwyd yn ei awgrymu.
Maeth enterol cyfyngedig
Mae deiet fformiwla hylif, a gymerir yn gyfyngedig am chwech i wyth wythnos, yn ysgogi gwelliant mewn clefyd Crohn pediatrig ar gyfraddau cyfatebol i gorticosteroidau — heb yr atal twf sy'n gwneud steroidau'n broblematig mewn plant. Mae'n driniaeth linell-gyntaf mewn llawer o ymarfer pediatrig Ewropeaidd.
Mae pam mae'n gweithio'n wirioneddol aneglur. Mae esboniadau posibl yn cynnwys dileu antigenau deietegol, newid y microbiom, dileu ychwanegion bwyd, a darparu maeth cyflawn yn syml. Mae'r ansicrwydd hwnnw'n bwysig, oherwydd dyna pam nad oes neb yn gallu dweud wrthych pa rannau o ddeiet normal i'w dileu i gael yr un effaith.
Maeth enterol ar gyfer ysgogi gwelliant
Ffibr: dadleuol, ac nid yn y ffordd mae pobl yn ei dybio
Archwiliodd adolygiad systematig a meta-ddadansoddiad ddeietau uchel-ffibr yng nghlefyd Crohn [2]. Mae'r darlun yn fwy ffafriol na'r hyn roedd y cyngor traddodiadol isel-weddillion yn ei awgrymu — ond gydag eithriad pwysig sy'n cael ei golli mewn crynodebau.
Mewn clefyd gweithredol gyda chulhad, gall ffibr swmpus, anhydawdd ysgogi rhwystr. Problem fecanyddol yw honno, nid un llidiol, a dyna pam mae cyngor cyffredinol yn methu. Mewn clefyd tawel heb gulhad, mae'r dystiolaeth ffibr gyffredinol yn berthnasol — gweler ein gwefan tystiolaeth coluddyn-ymennydd.
Popeth arall
| Dull | Tystiolaeth | Dyfarniad |
|---|---|---|
| Maeth enterol cyfyngedig | Cryf, clefyd Crohn pediatrig | Triniaeth wirioneddol |
| Deiet Eithrio Clefyd Crohn | Tystiolaeth ar hap sy'n tyfu | Addawol |
| Patrwm Môr y Canoldir | Iechyd cyffredinol, cyfyngedig yn benodol i IBD | Diofyn rhesymol |
| Isel-FODMAP | Rhyddhad symptomau, nid llid [4] | Symptomatig yn unig |
| Deiet Carbohydrad Penodol | Cyfyngedig, cymysg | Heb ei sefydlu |
| Heb glwten heb glefyd coeliag | Dim tystiolaeth benodol i IBD | Heb ei ddynodi |
| Dileu yn seiliedig ar brofi IgG | Dim dilysrwydd | Osgoi |
Mae rhes FODMAP yn haeddu pwyslais oherwydd mai hi yw'r un sy'n cael ei chamddeall fwyaf. Gall deietau isel-FODMAP leihau chwyddo a phoen mewn IBD yn ystyrlon — ac nid ydynt yn lleihau llid. Mae teimlo'n well tra bod llid yn parhau'n ganlyniad wirioneddol beryglus, a dyna pam mae'n rhaid i reolaeth ddeietegol wedi'i harwain gan symptomau eistedd ochr yn ochr â monitro gwrthrychol yn hytrach na'i ddisodli.
Bwydydd sy'n sbarduno
Mae adolygiad systematig o anoddefiad bwyd a deietau eithrio mewn IBD yn canfod yr hyn mae cleifion eisoes yn ei wybod: mae sbardunau'n real, yn unigol, ac yn anghyson rhwng pobl. Nid oes rhestr ddilysedig. Yr unig ddull dibynadwy yw tracio personol systematig — sy'n broblem data yn hytrach na phroblem gwybodaeth, a'r rheswm bod ap sganio sy'n dysgu eich patrwm eich hun yn fwy defnyddiol na thaflen wedi'i hargraffu.
Cwestiynau cyffredin
Alla i drin fy IBD gyda deiet yn unig?
Ddylwn i osgoi ffibr?
Ydy profion gwaed anoddefiad bwyd yn helpu?
Ydy deiet isel-FODMAP yn werth ei drio?
